dc.description.abstract	การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อพัฒนารูปแบบการประเมินสภาวะสุขภาพตามวัฒนธรรมของผู้รับบริการข้ามวัฒนธรรม ตามความคิดเห็นของผู้เชี่ยวชาญ โดยใช้เทคนิคเดลฟาย ผู้ให้ข้อมูลคือผู้เชี่ยวชาญ จำนวน 18 คน ได้มาจากการคัดเลือกแบบเจาะจง 6 คน และวิธีบอกต่อ 12 คน ซึ่งมีคุณสมบัติเป็นผู้เชี่ยวชาญทางการพยาบาล มีการศึกษาระดับปริญญาโทขึ้นไป มีประสบการณ์การดูแลผู้รับบริการข้ามวัฒนธรรมอย่างน้อย 5 ปี และ/หรือ มีผลงานวิชาการเกี่ยวกับการพยาบาลข้ามวัฒนธรรมปรากฏในช่วง 5 ปีย้อนหลัง (พ.ศ. 2555 – พ.ศ. 2560) เครื่องมือที่ใช้ในการวิจัย ได้แก่ แนวทางการสัมภาษณ์ที่ผู้วิจัยสร้างขึ้น การดำเนินการวิจัยแบ่งเป็น 2 ขั้นตอน (1) การสัมภาษณ์ผู้เชี่ยวชาญ จำนวน 3 รอบ โดยรอบที่ 1 เพื่อระบุองค์ประกอบ และมิติทางวัฒนธรรมที่ส่งผลต่อสภาวะสุขภาพของผู้รับบริการ แล้วนำข้อมูลไปจัดกลุ่มทำเป็นแบบสอบถามแบบประมาณค่า 5 ระดับ รอบที่ 2 นำแบบสอบถามแบบมาตรประมาณค่าที่ได้จากรอบที่ 1 ไปให้ผู้เชี่ยวชาญตอบแบบสอบถามแล้วนำข้อมูลที่ได้มาวิเคราะห์หาค่ามัธยฐานและค่าพิสัยระหว่างควอไทล์ และรอบที่ 3 นำข้อมูลในรอบที่ 2 มาสรุปแล้วนำไปให้ผู้เชี่ยวชาญยืนยันความคิดเห็นอีกครั้ง หลังจากนั้นนำข้อมูลที่ได้มาวิเคราะห์เพื่อหาข้อสรุป โดยกำหนดค่าที่ยอมรับได้ต้องมีค่ามัธยฐานเท่ากับหรือมากกว่า 3.50 และมีค่าพิสัยระหว่างควอไทล์น้อยกว่า 1.50 และ (2) พัฒนารูปแบบการประเมินสภาวะสุขภาพตามวัฒนธรรมของผู้รับบริการข้ามวัฒนธรรม โดยสอบถามความคิดเห็นจากพยาบาลวิชาชีพ ผู้เชี่ยวชาญให้ความหมายของสภาวะสุขภาพตามมิติทางวัฒนธรรม คือ การดำเนินกิจกรรมในการดำรงชีวิตของผู้รับบริการ ทั้งด้านร่างกาย จิตใจ สังคม และจิตวิญญาณ แตกต่างกันตาม วิถีชีวิต ความเชื่อ ค่านิยม โดยสรุปเป็น รูปแบบการประเมินสภาวะสุขภาพตามวัฒนธรรมของผู้รับบริการข้ามวัฒนธรรม ประกอบด้วย 4 องค์ประกอบ 7 มิติ คือ (1) องค์ประกอบด้านจิตวิญญาณ มี 1 มิติ คือ มิติด้านค่านิยมและความเชื่อของผู้รับบริการ (2) องค์ประกอบด้านสังคม มี 1 มิติ คือ มิติด้านองค์กรทางสังคม (3) องค์ประกอบด้านจิตใจ มี 3 มิติ คือ มิติด้านการสื่อสาร มิติด้านการเปิดพื้นที่ส่วนตัว และมิติด้านเวลา และ (4) องค์ประกอบด้านร่างกาย มี 2 มิติ คือ มิติด้านการเปลี่ยนแปลงส่วนบุคคล และ มิติด้านการควบคุมสิ่งแวดล้อม โดยผู้เชี่ยวชาญมีความเห็นว่าองค์ประกอบทั้ง 4 องค์ประกอบ และมิติทางวัฒนธรรม 6 มิติ มีความสำคัญและความเหมาะสมเพื่อใช้ในการประเมินสภาวะสุขภาพตามวัฒนธรรมของผู้รับบริการข้ามวัฒนธรรมในระดับมากที่สุด ยกเว้นมิติด้านการควบคุมสิ่งแวดล้อม ผู้เชี่ยวชาญมีความเห็นว่ามีความสำคัญและความเหมาะสมในระดับมาก การวิจัยครั้งนี้ทำให้ได้รูปแบบการประเมินสภาวะสุขภาพตามวัฒนธรรมของผู้รับบริการ ส่งเสริมในการได้มาซึ่งข้อมูลสภาวะสุขภาพของผู้รับบริการอย่างครบถ้วนเป็นองค์รวม พยาบาลวิชาชีพสามารถนำข้อมูลที่ไปวางแผนการพยาบาล และให้การพยาบาลได้อย่างมีประสิทธิภาพยิ่งขึ้น	th_TH

dc.description.abstractalternative	This research aimed to develop cultural health assessment model relying on expertise opinion by using Delphi technique. The subject were 18 nursing expertise, graduated master degree, having 5 years’ experience in cross cultural nursing, and/or having academic work about cross cultural nursing in five years form 2012 - 2017. They were 18 recruited by purposive sampling, and 12 by the snowball technique. The research instrument was interview guidelines which created by the researchers. There were 2 phases of research procedures; (1) interviewed the 3 rounds of experts. The first round aimed to identify the cultural dimensions affecting customer’ health, and then the data were categorized to be 5 rating scale items. The second round, experts were asked to complete the questionnaires and then analyzed the data by median and interquartile range. The third round, the results were confirmed by expert agreements and analyzed again by median and interquartile range with acceptable median ranked ≥ 3.50 and acceptable interquartile ranked <1.50. (2) to develop the cultural health assessment model for transcultural clients by contribution from the professional nurses. In expert’s perspectives, the definition of cultural health status was action or physically, mentally, socially and spiritually life styles which are different from the way of life, beliefs, values. The cultural health assessment model included 4 elements and 7 dimensions. First was spiritual element which included 1 dimension; the client’s values and beliefs. Second was social element which has 1 dimensions; social enterprise. Third, psychological element has 3 dimensions including communication personal space, and time dimension. Forth, physical element has 2 dimensions including: personal change and environmental control dimensions. The experts assumed that the four elements and six dimensions are important and appropriate for cultural health status assessing at the highest level but the environmental control dimensions was very important at high level. This research provided a model in order for assessing all aspects of the cultural health status of cross-cultural clients and professional nurses would gain knowledge to apply for efficiency nursing care plan and nursing practice.	en
