dc.description.abstract	การศึกษาครั้งนี้เป็นการวิจัยคุณภาพเพื่อบรรยายประสบการร์อาการหายใจลำบากในผู้ป่วยภาวะหัวใจล้มเหลว คัดเลือกผู้ให้ข้อมูลแบบเจาะจง ซึ่งเป็นผู้ป่วยที่มีอาการหายลำบากจากภาวะหัวใจล้มเหลวที่เข้ารับการรักษาในโรงพยาบาลตาคลี จังหวัดนครสวรรค จำนวน 10 ราย เก็บรวบรวบข้อมูลใช้วิธีการสัมภาษณ์เชิงลึก การสังเกตและการจดบันทึกภาคสนาม วิเคราะห์ข้อมูลโดยการวิเคราะห์เนื้อหา ผลการวิจัย พบประเด็นหลัก 3 ประเด็น ดังนี้ คือ ประเด็นหลักที่ 1) ความรู้สึกต่ออาการหายใจลำบาก พบ 2 ประเด็นย่อย เหนื่อยเหมือนจะตายและรู้สึกอยากตาย ประเด็นหลักที่ 2) ผลกระทบจากอาการหายใจลำบาก พบ 4 ประเด็นย่อย คือ ทำงานได้ไม่ดหมือนเดิม การปฏิบัติกิจวัตรประจำวันลดลง เป็นภาระของครอบครัว และผลกระทบด้านจิตใจ ได้แก่ กลัวตาย ท้อแท้ในชีวิต และความมีคุณค่าในตนเองลดลง ประเด็นหลักที่ 3) การจัดการอาการหายใจลำบากพบ 2 ประเด็นย่อย คือ การจัดการอาการด้วยตนเองและการจัดการอาการด้วยบุคคลอื่นโดยบุคคลภายในครอบครัวและบุคลากรทางสาธารณสุข ผลการวิจัยแสดงให้เห็นว่า พยาบาลควรทำความเข้าใจผู้ป่วยในเชิงลึกและนำข้อมูลที่ได้มาเป็นพื้นฐานของการวางแผนการพยาบาลให้กับผู้ป่วยภาวะหัวใจล้มเหลวได้อย่างมีคุณภาพมากยิ่งขึ้น	th_TH

dc.description.abstractalternative	The purpose of this qualitative study was to describe experiences of dyspnea in patients with heart failure. Ten participants with heart failure admitted to the Takhli hospital in Nakhon Sawan province were purposively selected. Data were collected by using in-depth inter-view, observation and field note. Content analysis method was used to analyze the data. The result found three themes. First, feeling of dyspnes which consisted of 2 categories: Tired like dying and feel like to die. Second, impacts of dyspnes which consisted of 4 categories: Being unable to work as usual, decreasing ability for daily living activities, feeling of families' burden, and psychological impact demonstrating with fear of dying, feeling of downhearted living, and low self-esteem. Third, symptom management of dyspnea which consisted of 2 categories: Self-managing symptom and managing symptoms by others including family members and health care providers. The results of this study shows that nurses should deeply understand the patients and use the results as a base for a more effective nursing care plan for the patients with heart failure.	en
