dc.description.abstract	การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อพัฒนารูปแบบการพัฒนาชุมชนและครอบครัวต้นแบบเพื่อดูแลผู้สูงอายุแบบบูรณาการ กลุ่มตัวอย่างประกอบด้วยผู้สูงอายุจำนวน 30 คน และผู้ดูแลจำนวน 30 คน ในจังหวัดปราจีนบุรี เก็บรวบรวมข้อมูลเชิงปริมาณและเชิงคุณภาพด้วยแบบสัมภาษณ์ วิเคราะห์ข้อมูลเชิงปริมาณด้วยค่าสถิติ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน paired t-test และวิเคราะห์ข้อมูลเชิงคุณภาพด้วยการวิเคราะห์เชิงเนื้อหา ผลการวิจัยทำให้ได้รูปแบบการพัฒนาชุมชนและครอบครัวต้นแบบเพื่อดูแลผู้สูงอายุ แบบบูรณาการในโรงพยาบาลส่งเสริมสุขภาพระดับตำบล จังหวัดปราจีนบุรี ที่เป็นระบบและมีประสิทธิผล ประกอบด้วย การศึกษาสถานการณ์ ความต้องการ การรับรู้ความสามารถตนเองในการดูแลสุขภาพของผู้สูงอายุ การกำหนดแนวในการพัฒนารูปแบบการดูแลสุขภาพ ผู้สูงอายุโดยใช้การเปรียบเทียบ Benchmarking Community based service และเศรษฐกิจพอเพียง การดำเนินงานตามแผนในโรงพยาบาลส่งเสริมสุขภาพระดับตำบล จังหวัดปราจีนบุรี และประเมินผลการดำเนินงาน เมื่อวัดผลผู้สูงอายุในด้านการรับรู้คุณค่าในตนเอง และพฤติกรรมสุขภาพของผู้สูงอายุ พบว่า คะแนนเฉลี่ยการรับรู้คุณค่าในตนเอง และพฤติกรรมสุขภาพของผู้สูงอายุ ระหว่างก่อนการทดลองและหลังการทดลองพบว่าดีขึ้น และมีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ (p<0.05) และเมื่อประเมินผลการดำเนินงานในผู้ดูแลในด้านการรับรู้คุณค่าในตนเองของผู้ดูแลและพฤติกรรมการดูแลผู้สูงอายุของผู้ดูแล พบว่า คะแนนเฉลี่ย การรับรู้คุณค่าในตนเองของผู้ดูแล และพฤติกรรมการดูแลผู้สูงอายุของผู้ดูแล ระหว่างก่อนการทดลองและหลังการทดลอง พบว่า ดีขึ้น และมีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ (p<0.05) ข้อเสนอแนะจากการวิจัยควรนำการเปรียบเทียบ Benchmarking Community based service และเศรษฐกิจพอเพียง มาประยุกต์ในการพัฒนาชุมชนและครอบครัวต้นแบบ ภายใต้ความตระหนักและการสนับสนุนของหน่วยงานที่เกี่ยวข้อง	th_TH

dc.description.abstractalternative	This research aims to develop a model for community development and family care for the elderly master integrated. The sample consisted of 30 elderly and 30 caregivers in Prachin Buri province. The quantitative data was done by using the percentage, mean, standard deviation and paired t-test . The qualitative data was analyze by content analysis. The research results showed that the study derived an effective and systematic model of quality improvement in Tambol Health Promoting Hospital in Prachin Buri province. The model consisted of sequentially integrated tools and activities including situation analysis, elderly need, self-efficacy, benchmarking process, community based service, sufficient economy , implementation of action plan and evaluation of the programs based on such plan. After applying the model, changes in self-esteem, and health behavior of elderly were measured and results indicated that the average score of self- esteem and health behavior of elderly before and after the experimented had increased, with statistical significance (p < 0.05). Changes were also found in caregivers in self-esteem and caring behavior before and after the experimented had increased, with statistical significance (p < 0.05). It was also suggested that benchmarking processes, community based service and sufficient economy could be used as important tools in community development and family care with the comments and supports of the related organization.	en
